• Referral document from your family practitioner specialized physician
• Personal identification card
• Health insurance card; if cost coverage declaration from your health insurance

For physicians and medical/nursing staff

• Medical diagnoses, such as results of blood tests, x-rays, ECG/EEG results
• Allergy passport, inoculation passport, identification card for diabetics, pacemakers; radiations passport, etc.
• List of current medications and, if applicable, these medications in their original packaging
• Contact information/addresses of friends
• Nursing transfer sheet, if social services should handle your at-home care
• Patient disposition /powers of attorney

• Eye glasses
• Dental prosthetics
• Hearing aids, incl. replacement batteries
• Walking tools, e.g. Rollator, walking stick, etc.
• Customised compression socks

Clothing

• Slippers and solid shoes (one pair each)
